Requirements:

1. Completed visa application form. FILL OUT THIS FORM
2. You must ship your actual US passport (no photocopies). Your passport should remain valid for the next 6 months.
3. One passport photo (2×2 inches, color, with a plain light or white background).
4. A business letter of responsibility/guarantee is required to be typewritten on your company letterhead and addressed to the embassy, explaining nature of business to be performed, name and address of business reference in country to be visited, guarantee of return transportation and sufficient funds for the visit. This letter must be signed by an officer of the company other than the applicant.
5. Letter from Government Agency: If applying for a government contractor visa, a letter from the government agency or DoD authorization is required.
6. Payment of applicable fees. Business Visa: $350 (Embassy Fee) + $250 (UPV Fee) = $600 total.

• Same-day processing is not available for business visas.
