Step 1: Gather U.S. Passport Application Materials
1. Letter of Authorization is required: TPV Letter of Authorization Form
2. Two Passport Photos
- Must be 2×2 inches, in color, with a plain light or white background.
- Should show a full front view of the face.
3. Most Recent Passport
4. Official Name Change Documentation
(if applicable)
If the name on your current passport differs from your previous one, provide:
- Marriage certificate, divorce decree, or court order showing the name change.
- Must bear the official seal of the issuing authority.
- Uncertified copies
or notarized documents
are not accepted.
All documents will be returned with your passport.
5. Letter Requesting Early Renewal
(if applicable)
Include a letter addressed to the U.S. Department of State
explaining why you need your passport renewed early.
Valid reasons include:
- Dramatic changes in appearance
- Extended trips abroad
- Other urgent travel needs
6. Travel Evidence
Provide two copies
of one
of the following:
- Copy of your plane ticket showing foreign departure
- Copy of travel itinerary (from airline or travel agency with your name, travel date, and flight info)
- Business travel letter (if applicable)
7. DS-82 Government Renewal Application
Complete online using the official government form:
Print the completed form and sign.
